Just to throw yet another possible solution out there I had this problem a couple of weeks ago. It turns out that I didn't have the password for my email server saved. I'm not in front of my MBP right now, so I can't give you the step-by-step sequences, but as I recall, I went into preferences where it shows you your email accounts. At the bottom there was a little drop down box where you could edit your server lists. From there I recall clicking 'advaced' and that's where I could see that I didn't have a password entered. I have tried retyping the password. I wonder if the third point you made about typing in the User Name is part of the problem. The Cox network drops the @server part of the email address in establishing user names. So my user name is not the same as my email. Everything was ok up until 3 years ago, and somehow my email account decided to send out spam to hundreds of different people, who in turn complained to AOL. Now, even though I told them I had nothing to do with it, they seemed to have put some type of blocker on my accounts. When I try to transfer several different emails from one AOL account to another (mine) AOL account, it thinks that I am spamming people, and asks for my password.

When opening Outlook, the status at the bottom will say 'Connected to Exchange', but then a few seconds later the password prompt appears. The user's mailbox is migrated fully into 365 and the user account is also fully in sync with AzureAD/Local AD.
